Survey for Diaper Boys

Take the survey. The survey form is contained on one page.
Same as above, except that the survey is split into two separate forms. The split seems to be more reliable and is recommended for people with older systems with less memory (<64MB). If you choose to use this form, please be sure to fill out both forms!Only complete responses with both parts are counted and posted.
A downloadable plain text file that can be filled out and returned via email. This option is recommended for those who are either unable to use the online forms, do not wish to keep their computers online for the duration of filling out the survey, or for those who are otherwise under circumstances which do not allow for taking the online version.
